Advance rights-based mental health reform and integration into PHC: 1) Conduct public engagement and socialization for draft mental health legislative notes (roundtables, town-halls, CSO forums); 2) Co-design PHC-integrated care pathways and referral protocols (mhGAP-informed) and pilot in two polyclinics. 3) Enact mental health law and regulations; operationalize rights protections (advance directives, supported decision-making, complaints); start community mental health teams in priority clinics; train primary care teams; set crisis response and safe referral.; 4) Implement rights-based model in primary care and community; expand community mental health teams nationwide; monitor involuntary admissions and rights complaints; embed peer support and family participation; publish “you said, we did” actions; independent review of rights compliance.

Establish an integrated geriatric/long-term care model linked to PHC and hospital services that includes a plan for the management of health risks and emergencies. 1) Define and approve the package of services for the new geriatric hospital (clinical services, rehab, social support, quality & safety). 2) Design and deliver training for interprofessional teams (geriatric medicine, rehabilitation, dementia care, IPC, patient safety). 3) Finalize design and launch a pilot of an integrated geriatric and long-term care model linking primary health care, hospital and community services; sign care-coordination standard operating procedures; train multidisciplinary teams (at least one hundred staff). 4) Scale the geriatric and long-term care model to all parishes; implement shared, person-centred care plans with referral and counter-referral captured in the electronic health record; monitor avoidable bed-days and readmissions; establish quality indicators and governance for the model.

Strengthen integrated networks and continuity of care across services: 1) Map and strengthen referral/counter-referral between PHC, geriatric hospital and community services; implement shared protocols and feedback loops. 2) Standardize referral and counter-referral pathways across services; establish a care-navigator function in select regions; hold monthly multidisciplinary case conferences; implement a hospital-to-home discharge planning protocol. 3) Institutionalize integrated network governance with service-level agreements and key performance indicators; deploy continuity-of-care dashboards; expand the care-navigator function nationwide; conduct an annual network performance review with community feedback and published actions.
