For the physical and mental health of elders, we believe that we should address the isolations of elders in staying in their home all the time. To encourage and support them to go out and have active daily lives, we provide our “The city that nudge you to go out” policy package. We have expanded our community bus system, with which supports elders can go out easily. We installed all-age stretching equipment in more than 100 parks all over the city. To provide the daily destination for elders, we opened the new city library, in which the elders can go and have relaxed and meaningful time every day.
We also make outreaching efforts to the elders with high risk of malnutrition, low weight, and frail. Our public health nurses and registered dieticians regularly visit homes of elders with elders with high risks. They talk face to face with those people, build rapport, and help the improvement of lifestyles and health habits over six months. The project has made great success, both in improving health vitals and lifestyles of those elders and in cutting medical and long term elder care costs.
Both our “The city that nudge you to go out” policy package and the hove visit project by public health nurses and registered dieticians have been awarded, nationally and internationally.
With the rapid population greying, we are expanding our age-friendly related projects further. We are building the community-based elder care system, which provides medical, long-term elder care and life supports services. We are also expanding our support system for the dementia patients and their families.
