Establishment of Tai Po Dementia Care Link


Summary

The Salvation Army,  Alice Ho Miu Ling Nethersole Hospital and Tai Po Hospital have established the “Tai Po Dementia Care Link” on 1 June 2017. The “Tai Po Dementia Care Link” joins together 30 NGOs & local stakeholders in the Tai Po district, with the goal of helping to address the challenges associated with dementia with multidisciplinary, cross-sectorial and district-based collaboration.

Mission:

  • To increase the public awareness of dementia and to consolidate the power of local stakeholders in order to build a Dementia Friendly Community in Tai Po.

Objective:

  • To consolidate the partnership of local stakeholders and to work out the action plan on a dementia-friendly community together.
  • To facilitate the exchange of information, work experience and resources among district partners.
  • To increase public education on dementia care and brain health promotion.
  • To organize various programmes/projects in order to strengthen dementia care services in the district in view of community needs.

Action plan

  • The “Dementia Supporter Program” is a new initiative to train the general public to enhance their awareness, understanding and basic knowledge of dementia. The target is to recruit at least 2,000 supporters by the end of 2019. As of October 2017, there’re over 200 supporters joined the programme.
  • In response to The World’s Alzheimer Day, Tai Po Dementia Care Link Launching Ceremony was held on 21 September 2017 in order to engage over 240 local leaders and stakeholders to declare to share the mission of building Dementia Friendly Community.
  • Carer café and training programs are organized for dementia caregivers monthly so as to enhance their self-efficacy and to reduce the caregiving burden.
  • A district forum was held on 27 November 2017. Experts and local leaders were invited to share their views on how to build a dementia-friendly community in the Tai Po district.
