5. Models of care (incl. service packages)

 Defined as models of care that promote high-quality, people-centred primary care and essential public health functions as the core of integrated health services throughout the course of life. Focusing on what models of care will best support PHC orientation of health systems for Universal Health Coverage and Sustainable Development Goals.


KEY QUESTIONS TO SUPPORT PHC-ORIENTED APPROACH TO ADAPTING NATIONAL HEALTH PLAN:
• What PHC policy commitments have been made  to guide development of comprehensive integrated health services (including combining public health and primary care)?
• Have PHC policy frameworks been updated for models of care to include role of  all relevant health workers (e.g. community health workers) in PHC, appropriate technologies, multi disciplinary teams and two way referral systems?
• Have adequate resources been mobilised to support capacity building on models of care, adoption of appropriate technologies and integrated Health Information Systems and for implementation research?
