Abstract
An optimal planning model for rehabilitation of water networks is presented. Capital costs (replacement, rehabilitation and repairing costs), benefits (by the reduction of pumping cost and leakage cost), and hydraulic reliability are used for making an optimal decision for the rehabilitation plan of a water pipe network. KYPIPE is used for checking the hydraulic reliability. A multi-objective optimization model is successfully developed in this study. And the task is tackled using a new meta-heuristic algorithm, Harmony Search, for solving a large optimization problem to which conventional optimization techniques are poorly suited. Five different models with different objective functions are developed and tested according to various conditions considered in this study. These models provide more options for the rehabilitation of pipe network systems compared to previously suggested models in the literature.

Abstract
Homology of ars (arsenic-resistance system) genes was examined among the indigenous bacteria isolated from the soils and sediments of two abandoned Au mines, which are highly contaminated with arsenic. The DNA and amino acid sequence homology of the ars determinants were investigated using an ars genotype. The isolated strains showed As(III)-oxidation ability contained arsAB genes encoding the efflux pump as well as arsR and arsD regulator genes. The arsR and arsD leader gene are required for an arsenic resistance system when the high-homology genes (arsR; pI258 52.09% and arsD; Shewanell sp. 42.33%) are controlled by the ars inducer-independent regulatory amino acid sequence. These leader gene were observed under weak acidic conditions in the Myoung-bong (pH; 5.0 to 6.0) and Duck-um (pH; 4.0 to 7.0) mines. In addition, the strains with the ability of As(V)-reduction involved the arsC gene homologues, as in the strain OW-16 (Pseudomonas putida). The arsenic-resistance genes in the isolated indigenous bacteria showed varying degrees of amino acid similarity to the homologous genes found in the database (GenBank) such as P. putida KT2440: 39-53% for arsR, 22-42% for arsD, 16-84% for arsA, 26-45% for arsB, 17-44% for arsAB, 37-41% for arsC, and 14-47% for arsH. These findings suggested that the function of the various ars gene in indigenous bacteria existing in weakly oxidative conditions may be the key factor for redox mechanisms and biogeochemical systems in arsenic contaminated soils. © KSBB.

Abstract
Four adult men fell into an artificial lake which was being used to raise flatfish, after a water pipe had been connected to a tube allowing seawater to flow into the lake. Forensic autopsies were carried out on three of the four men, who died soon after the incident. From autopsy findings, the cause of death was diagnosed to be suffocation after aspirating seawater in the three victims. To clarify why the men fell into the lake, a chemical analysis for hydrogen sulfide was carried out using the extractive alkylation technique combined with gas chromatography/mass spectrometry. The sulfide was detected as its derivative, bis(pentafluorobenzyl)sulfide, in body tissues taken from all the victims, and the concentration of hydrogen sulfide gas at the scene was estimated as having been nearly fatal.

Abstract
BACKGROUND: The size of the west African Ebola virus disease outbreak led to the urgent establishment of Ebola holding unit facilities for isolation and diagnostic testing of patients with suspected Ebola virus disease. Following the onset of the outbreak in Sierra Leone, patients presenting to Connaught Hospital in Freetown were screened for suspected Ebola virus disease on arrival and, if necessary, were admitted to the on-site Ebola holding unit. Since demand for beds in this unit greatly exceeded capacity, we aimed to improve the selection of patients with suspected Ebola virus disease for admission by identifying presenting clinical characteristics that were predictive of a confirmed diagnosis. METHODS: In this retrospective cohort study, we recorded the presenting clinical characteristics of suspected Ebola virus disease cases admitted to Connaught Hospital's Ebola holding unit. Patients were subsequently classified as confirmed Ebola virus disease cases or non-cases according to the result of Ebola virus reverse-transcriptase PCR (EBOV RT-PCR) testing. The sensitivity, specificity, positive predictive value, negative predictive value, and likelihood ratio of every clinical characteristic were calculated, to estimate the diagnostic accuracy and predictive value of each clinical characteristic for confirmed Ebola virus disease. RESULTS: Between May 29, 2014, and Dec 8, 2014, 850 patients with suspected Ebola virus disease were admitted to the holding unit, of whom 724 had an EBOV RT-PCR result recorded and were included in the analysis. In 464 (64%) of these patients, a diagnosis of Ebola virus disease was confirmed. Fever or history of fever (n=599, 83%), intense fatigue or weakness (n=495, 68%), vomiting or nausea (n=365, 50%), and diarrhoea (n=294, 41%) were the most common presenting symptoms in suspected cases. Presentation with intense fatigue, confusion, conjunctivitis, hiccups, diarrhea, or vomiting was associated with increased likelihood of confirmed Ebola virus disease. Three or more of these symptoms in combination increased the probability of Ebola virus disease by 3.2-fold (95% CI 2.3-4.4), but the sensitivity of this strategy for Ebola virus disease diagnosis was low. In a subgroup analysis, 15 (9%) of 161 confirmed Ebola virus disease cases reported neither a history of fever nor a risk factor for Ebola virus disease exposure. INTERPRETATION: Discrimination of Ebola virus disease cases from patients without the disease is a major challenge in an outbreak and needs rapid diagnostic testing. Suspected Ebola virus disease case definitions that rely on history of fever and risk factors for Ebola virus disease exposure do not have sufficient sensitivity to identify all cases of the disease. Abstract
The incidence of esophageal adenocarcinoma (EAC) has risen sharply in western countries over the past 4 decades. This type of cancer is considered to follow a transitional process that goes from gastro-esophageal reflux disease (GERD) to Barrett's esophagus (BE, a metaplastic condition of the distal esophagus), a precursor lesion and ultimately adenocarcinoma. This spectrum of GERD is strongly predominant in males due to an unidentified mechanism. Several epidemiologic studies have described that the prevalence of GERD, BE and EAC in women is closely related to reproductive status, which suggests a possible association with the estrogen level. Recently, we revealed in an in vivo study that the inactivation of mast cells by the anti-inflammatory function of estrogen may account for the gender difference in the GERD spectrum. Other studies have described the contribution of female steroid hormones to the gender difference in these diseases. Estrogen is reported to modulate the metabolism of fat, and obesity is a main risk factor of GERDs. Moreover, estrogen could confer esophageal epithelial resistance to causative refluxate. These functions of estrogen might explain the approximately 20-year delay in the incidence of BE and the subsequent development of EAC in women compared to men, and this effect may be responsible for the male predominance. However, some observational studies demonstrated that hormone replacement therapy exerts controversial effects in GERD patients. Nevertheless, the estrogen-related endocrine milieu may prevent disease progression toward carcinogenesis in GERD patients. The development of innovative alternatives to conventional acid suppressors may become possible by clarifying the mechanisms of estrogen.

Abstract
Aconitum heterophyllum is the source of the Tibetan medicinal drug bong- dkar. All parts of the plant may be used for the treatment of the diseases rim-tshad, mkhris-tshad, and tsha-ba of the small intestine. Bong-dkar is also valued as a useful remedy against snake and scorpion poison. Aconitum heterophyllum grows in the northwestern Himalayas at altitudes of 2000 to 4500 m. Morphological details of the plant and anatomical characteristics of the above-ground parts are given. The main anatomical characteristics are three types of unicellular hairs, unequally sized epidermis cells of the leaf and a sclerenchyma belt in the stem.

Abstract
The foramen ovale which is the fetal connection between the right and left atrium persists in about 30 % of the adult population. In the presence of a persistent foramen ovale (PFO) shunting of blood may occur from the right to the left atrium, and bubbles can reach the systemic circulation during or after the decompression phase of a dive with compressed air. Therefore, divers with PFO may have an increased risk to develop ischemic cerebral lesions and neurologic decompression sickness (DCS). Significant right-to-left shunting may be diagnosed using transcranial doppler ultrasound of the medial cerebral artery and echocardiography with echo contrast media and Valsalva provocation. However, there are no official guidelines concerning PFO screening in medical fitness exams for professional or recreational divers in Germany. Therefore, it remains in the diver's choice to be screened for PFO. Divers with a history of DCS should be monitored for PFO, especially when diving strictly adhered to decompression tables. Divers with PFO who refuse to stop diving after DCS should be advised to adhere to very save dive profiles.

Abstract
The aim of this work is to draw attention to possible injuries of the eye and the facial skeleton caused by firefighting sport. There was a group of 9 patients presented who were treated from 2006 to 2015 in the Department of Ophthalmology at the University Hospital in Pilsen and diagnosed with severe eyeball contusion after being hit by a jet of water and/or a water pipe. Three cases are presented in detail.

Abstract
The studies have shown that despite studying medicine the rate of tobacco smokers among students is still high. Moreover, in Poland the incidence of smoking is increasing among the youngsters. The study was designed to assess the rate of smokers among the first-year students of two consecutive courses at the Medical University of Gdansk and define their attitudes towards tobacco smoking. The voluntary, multiple-choice questionnaires were distributed among 412 students. The rate of response was 100%. Twenty one percent of responders were current smokers (17% females and 28% men), whereas 7% declared previous smoking. There was no correlation between incidence of smoking among students and their parents (p=0.11). 61% of smokers declared the will to give up their habit and of those 94% were aware of the negative impact of cigarettes on their health. In the group which did not declare the will to quit smoking only 75% knew what the impact of cigarettes on their health was; p=0.02. Only 23% of smokers considered anti-nicotine therapy effective. Almost a half of smokers considered nicotine replacement therapy ineffective for them in quitting cigarette smoking. CONCLUSION: The rate of smokers among the first-year medical students is lower than in general population, but it is still relatively high. However more than a half of smokers wants to give up their habit. Of those who are not willing to, 25% is not aware of the cigarettes' impact on their health. These results call for further anti-nicotine actions.
