Abstract
OBJECTIVE: To investigate the influence of cigarette or sheesha smoking on first-trimester markers of Down syndrome. DESIGN: A prospective observational study. SETTING: Primary care centres and antenatal clinics of Maternity and Children Hospital, King Abdulaziz University Hospital and New Jeddah Clinic Hospital, Jeddah, Saudi Arabia. POPULATION: Women with a singleton pregnancy who were either nonsmokers (n = 1736) or cigarette smokers (n = 420) or sheesha smokers (n = 181). METHODS: Fetal nuchal translucency thickness (fetal NT), maternal serum free beta-human chorionic gonadotrophin (free beta-hCG) and pregnancy-associated plasma protein-A (PAPP-A) were measured at 11 weeks 0 days to 13 weeks 6 days of gestation in all women. Women were grouped according to smoking status, confirmed by maternal serum cotinine measurements, and analyte levels between groups were compared. MAIN OUTCOME MEASURES: Fetal NT, maternal serum free beta-hCG, PAPP-A and cotinine measurements. RESULTS: Compared with nonsmoking women, fetal NT was significantly increased and free beta-hCG and PAPP-A levels were significantly decreased in both cigarette and sheesha smokers. There were significant relationships between all three markers and the number of sheeshas consumed per day. CONCLUSIONS: Cigarette and sheesha smoking significantly affect first-trimester markers of Down syndrome (fetal NT, free beta-hCG and PAPP-A). Correction for this effect in women who smoke might improve the effectiveness of first-trimester screening for Down syndrome in these women. The underlying mechanism(s) relating smoking to the changes in first-trimester markers require further studies.

Abstract
Objectives and background: Hazard of smoking tobacco is believed to be minimized by smoking hubble-bubble (HB) instead of cigarettes. Our aims were to (i) develop an assay for estimating nicotine and cotinine; and (ii) evaluate the effect of smoking on respiratory and metabolic parameters in cigarette and HB smokers. Methods: Urine samples were collected from 152 volunteer smokers (75 cigarette and 77 HB) as well as from 16 healthy controls. We optimized an HPLC method for the determination of nicotine and cotinine. Subjects were asked to complete a chronic respiratory symptoms questionnaire and to undergo spirometry. Fasting blood samples were collected for the determination of their lipid profile. Results: The intra-assay coefficients of variation for nicotine and cotinine were 16.6% and 6.6%, respectively. The mean of cotinine in cigarette smokers (1321.4 ng/mL) was significantly (P = 0.008) higher than the mean cotinine (677.6 ng/mL) in HB smokers. The mean nicotine level in cigarette smokers (1487.3 ng/mL) was significantly (P < 0.0001) higher than the mean nicotine (440.5 ng/mL) in HB smoker. The urinary cotinine and nicotine levels of the control subjects were lower than the detection levels of the assay. The mean high-density lipoprotein cholesterol was lower in cigarette smokers (0.99 mmol/L) compared with HB smoker smokers (1.02 mmol/L) but this was not significant (P = 0.28). Spirometric values were comparable among the three groups but the chronic respiratory symptoms in the smoking groups appeared at an earlier age in the HB smokers compared with the cigarettes smokers (P < 0.05). Conclusion: Smoking HB does not reduce the risk of tobacco exposure and it's potentially harmful metabolites on health. © 2006 Asian Pacific Society of Respirology.

Abstract
To assess the prevalence of sheesha smoking and the personal, social and environmental factors associated with it. Subjects and A convenience sample of 761 students 261 male, 500 female] in the teacher training program of the Public Authority for Applied Education and Training in Kuwait City answered a 70-question survey regarding sheesha use. Data were analyzed using SPSS version 10. 24.6% of males and 5.5% of the females smoked sheesha. 49.2% of the male sheesha smokers smoked at least one bowl a day, as did 26.9% of the female sheesha smokers. The majority of sheesha smokers first started smoking sheesha at age 18 or older. Almost half were encouraged to smoke sheesha the first time by their friends. The majority of sheesha non-smokers had sheesha non-smokers as friends. Among the sheesha smokers, 59.2% of females and 61.3% of males said all or most of their friends smoked sheesha. Sheesha smokers were more likely than sheesha non-smokers to live in a home where there are other sheesha smokers. Half of the males and one-third of the females who smoked sheesha wanted to quit. The majority of sheesha smokers also smoked cigarettes. Teachers, including female teachers, were frequently seen smoking in their schools. As expected, sheesha smokers had more positive attitudes towards sheesha smoking and were less likely to believe in its harmful effects. This is the first known study of sheesha use among college students in Kuwait. Results suggest efforts to reduce sheesha smoking in this young population should: 1] help young people address pressures from peers, 2] reduce sheesha smoking at home and school environment, 3] counteract personal beliefs and attitudes that contribute to sheesha smoking, and 4] reinforce beliefs about the health risks of sheesha smoking

Abstract
Blood cadmium (B-Cd) and urinary cadmium (U-Cd) were assessed among non-smokers, cigarette smokers, sheesha smokers, and both cigarette and sheesha smokers. Urinary cotinine (U-cotinine), which is a metabolite of nicotine with a biological half-life of > 10 h was used as a marker of exposure to tobacco smoke. Results indicate that smoking sheesha is another source of tobacco exposure. B-Cd, U-Cd, and U-cotinine were significantly higher among cigarette and both cigarette and sheesha smokers. U-cotinine was associated only with B-Cd concentrations. Exposure to Cd from smoking cigarette or/and sheesha could effect some of the renal dysfunction indices. (C) 2000 Wiley-Liss, Inc.

Abstract
The relationships between haemoglobin concentration Hb], haematocrit HCT] and carboxyhaemoglobin COHb] levels were investigated in 186 sheesha, and 155 cigarette smokers and 132 non-smokers. Both mean Hb and HCT values were found to be significantly higher in sheesha smokers of all age groups p<0.001], heavy cigarette smokers aged 40 to 49 years p<0.05] and light plus heavy cigarette smokers aged 50-65 years. Carboxyhaemoglobin levels were higher in sheesha than cigarette smokers. Positive correlations were found between COHb levels and that of HCTvalues in both sheesha and cigarette smokers. In seven out of twelve sheesha smokers investigated, the red cell mass was normal while Plasma volume was decreased in ten subjects. It seems that relative rather than absolute polycythaemia is the rule in sheesha smokers. These findings cast doubt on the extent of the relationship between carbon monoxide and erythrocytosis

Abstract
A single carboxyhaemoglobin (COHb) estimation of late evening blood sample among non-smokers, cigarette smokers, and sheesha smokers was evaluated among Saudis. The COHb level in smokers of 15 to 40 cigarettes a day ranged between 0.7 and 10.3 with a mean value of 6.1 +/- 2.58 COHb. Values among sheesha smokers ranged between 6.5 and 13.9 with a mean value of 8.8 +/- 1.83, significantly higher than those of cigarette smokers (P less than 0.001) for a given degree of exposure to tobacco smoke.
