Abstract
CONTEXT: Exposure to environmental tobacco smoke is a health risk that is of concern to patrons and of particular concern to employees of restaurants and bars. OBJECTIVE: To assess environmental tobacco smoke exposure (using expired carbon monoxide levels) in non-smoking waiters before and after a normal day's shift and to compare pre-exposure levels with non-smoking medical students. DESIGN: An observational study. SETTING: Restaurants with more than 50 tables or 100 places in Sao Paulo. SUBJECTS: 100 non-smoking restaurant waiters and 100 non-smoking medical students in Sao Paulo, Brazil. MAIN MEASUREMENTS: Levels of expired carbon monoxide, measured with a Smokerlyser (Bedfont EC 50 Scientific), before and after a normal day's work. RESULTS: Waiters' pre-exposure expired carbon monoxide levels were similar to those of medical students, but after a mean of 9 hours exposure in the workplace, median levels more than doubled (2.0 ppm vs. 5.0 ppm, P <0.001). Post-exposure carbon monoxide levels were correlated with the number of tables available for smokers (Kendall's tau = 0.2, P <0.0001). CONCLUSIONS: Exposure to environmental tobacco smoke is the most likely explanation for the increase in carbon monoxide levels among these non-smoking waiters. These findings can be used to inform the ongoing public health debate on passive smoking.

Abstract
This study intends to give recommendations to the management of Paraty fishery in Brazil through an interplay of local and scientific knowledge. In particular, the objectives are the following: 1) to describe the Paraty fishery; 2) to compare the fishermen's local ecological knowledge with recorded fish landings and previous studies in Paraty; 3) to combine the data on local fishing and on local/Caicara livelihoods with the SES (social-ecological systems) Model. The methods include a systematic survey of fishing in Tarituba and Praia Grande, which are located in the northern end and the central part of the Paraty municipality, respectively. For four days each month, systematic data on catches at landing points were collected, as well as macroscopic gonad analysis data for the fishes Centropomus parallelus and C. undecimalis (snook, robalo), Epinephelus marginatus (grouper, garoupa), Scomberomorus cavalla (King mackerel, cavala), and Lutjanus synagris (Lane snapper, vermelho). Spring and summer are important seasons during which some species reproduce, and the integration of fishing periods for some target species could assist in fishing management through the use of closed seasons. Fishermen could obtain complementary earnings from tourism and from the "defeso system" (closed season including a salary payment) to conserve fishing stocks. The SES model facilitates an understanding of the historical context of fishing, its economic importance for local livelihoods, the constraints from conservation measures that affect fishermen, and the management processes that already exist, such as the defeso. If used to integrate fishing with complementary activities (tourism), such a system could improve the responsibility of fishermen regarding the conservation of fish stocks.

Abstract
OBJECTIVE: To describe the prevalence and user profile of electronic cigarettes among Spanish adults and evaluate the potential dual use of these devices with combustible or conventional tobacco in 2014 in Spain. METHODS: Cross-sectional study of a representative sample of the Spanish adult (16-75 years old) population (n=1,016). A computer-assisted telephone survey was conducted in 2014. The prevalence and 95% confidence intervals (95% CI) for the use of electronic cigarettes stratified by gender, age, tobacco consumption and social status were calculated. The sample was weighted and a logistic regression model adjusted to obtain the crude odds ratios (OR) adjusted by gender, age and social status. RESULTS: 10.3% (95% CI: 8.6-12.4) of the Spanish adult population stated being ever users of electronic cigarettes (2% current users, 3.2% past users and 5.1% experimental users). Among current electronic cigarette users, 57.2% also smoked combustible or conventional tobacco, 28% had never smoked and 14.8% were former smokers. The prevalence of electronic cigarette use was higher in the younger population (adjusted OR=23.8; 95% CI: 2.5-227.7) and smokers of combustible tobacco (adjusted OR=10.1; 95% CI: 5.8-17.5). CONCLUSIONS: The use of electronic cigarettes in Spain is scarce and is most prevalent among young people and tobacco smokers. Nevertheless, one out of four current electronic cigarette users have never smoked. Hence, the regulation of these devices should be reinforced to avoid a possible gateway to nicotine products among never smokers.

Abstract
OBJECTIVE: To describe the nicotine dependence and readiness to quit smoking in the smoker population. METHODS: Cross-sectional study on a representative sample of the Spanish population of >=18 years old. We gathered information by means of telephone interviews conducted between June and July of 2006. We studied the nicotine dependence with the Fagerstrom Test for Nicotine Dependence (FTND) and the readiness to quit according to the stages of change from the Transtheoretical Model in a sample of cigarette smokers. RESULTS: 22.5% of participants (95% CI: 20.9-24.2%) smokers cigarettes. They smoked an average of 14.4 cigarettes per day (standard deviation 9.15) and the mean FTND score was 2.8, with no differences by the stages of change. 64.3% (95% CI: 60.3-68.2%) of smokers were in the precontemplation stage, 25.4% (95% CI: 21.8-28.9%) in contemplation, and 10.4% (95% CI: 7.9-12.9%) in preparation, with no differences by sex. The most nicotine dependent smokers (FTND>=6) had mainly primary studies, started to smoke at earlier ages, and smoked more cigarettes per day. CONCLUSIONS: Most Spanish smokers have low nicotine dependence and are in precontemplation stage. Smoking cessation programmes should be addressed to reduce dependence, help smokers to progress through the stages of change, and, consequently, reduce the prevalence of smokers in the population.

Abstract
Amphotericin B (AMB), micafungin, and caspofungin MICs, minimal fungicidal concentrations, and time-killing curves were determined in the presence and absence of 10% inactivated serum. AMB was the only agent with consistent killing activity (time required to achieve 99.9% of growth reduction was 2.1 to 3.2 h). The presence of serum enhanced caspofungin activity but lowered those of micafungin and AMB.

Abstract
Invasive infections caused by Candida spp. are increasing worldwide and are becoming an important cause of morbidity and mortality in immunocompromised patients. A large number of manifestations of candidiasis are associated with the formation of biofilms on inert or biological surfaces. Candida spp. biofilms are recalcitrant to treatment with conventional antifungal therapies. The aim of this study was dual 1) to determine the prevalence of biofilm producers among clinical isolates from catheter (16 C. albicans ) and blood culture (2 C. albicans and 30 C. tropicalis), and 2) to determine the activity of amphotericin B and anidulafungin against C. albicans and C. tropicalis biofilms of 24 and 48 hours of maturation. Biofilms were developed using a 96-well microtitre plate model and production and activity of antifungal agents against biofilms were determined by the tetrazolium (XTT) reduction assay. Of catheter and blood isolates, 62.5 and 56.25%, respectively, produced biofilms. By species, 68.42% of C. albicans and 53.33% of C. tropicalis were biofilm producers. C. albicans biofilms showed more resistance to amphotericin B and anidulafungin than their planktonic counterparts. Complete killing of biofilms was never achieved, even at the highest concentrations of the drugs tested. Anidulafungin displayed more activity than amphotericin B against C. albicans biofilms of 24 hours of maturation (GM MIC 0.354 vs. 0.686 microg/ml), but against C. tropicalis biofilms amphotericin B was more active (GM MIC 11.285 vs. 0.476 microg/ml). In contrast, against biofilms with 48 hours maturation, amphotericin B was more active against both species.

Abstract
OBJECTIVE: To discover the evolvement of Primary Care doctors' tobacco consumption in Guipuzcoa and their attitudes to tobacco dependency. DESIGN: A descriptive study of a crossover type. October 1992. SETTING: Primary Health care. PARTICIPANTS: 381 general physicians and paediatricians from Guipuzcoa province. MEASUREMENTS AND MAIN RESULTS: A self-filled questionnaire was distributed at Health Centres for subsequent return by mail. The reply rate was 60.89%, 42.3% were smokers; 33.6%, ex-smokers. 43% of smokers smoked in the Centre; 2% in front of patients. We found significant differences (p < 0.05) with a study carried out in 1988, where doctors who smoked were 52.4%, ex-smokers 19% and doctors who smoked in front of patients, 18.2%. 45.7% of women smoked, 39.2% of men. 78% of the doctors under study stated that they questioned patients about tobacco consumption. 95.2% did so when patients had specific pathologies. CONCLUSIONS: There has been a significant decline in tobacco smoking by doctors in Guipuzcoa. There are more women than men smokers. There is a higher proportion of ex-smokers among doctors than in the population as a whole. There has been a positive change of attitude as to smoking in front of patients. Centres should become more involved in the struggle against tobacco by offering to set up dependency counselling groups.

Abstract
INTRODUCTION: The present study examined the reduction in exposure to tobacco smoke in German hospitality venues following the implementation of a partial smoking ban by measuring the indoor air concentration of PM(2.5) in 2005 and 2009, that is, before and after the legislation was implemented. METHODS: The concentration of respirable suspended particles (PM(2.5)) in the indoor air of German hospitality venues was measured using a laser photometer (AM510). The prelegislation sample from 2005 included 80 venues of which 58 could be revisited in 2009. After replenishment, the postlegislation sample consisted of 79 venues. RESULTS: Compared with the prelegislation measurement, the concentration of PM(2.5) in hospitality venues was reduced significantly after introduction of the smoke-free legislation. The median mass concentration of PM(2.5) was reduced by 87.1% in coffee bars, by 88.7% in restaurants, by 66.3% in bars, and by 90.8% in discotheques. Notably, legal exemptions to the smoking ban are an issue: At the postlegislation measurement in 2009, the mass concentrations of PM(2.5) were substantially higher in venues allowing smoking in the whole venue or in a designated smoking room than in completely smoke-free venues. CONCLUSIONS: The German smoke-free legislation significantly reduced the levels of respirable suspended particles in the indoor air of hospitality venues, benefiting the health of employees and patrons alike. But legal exemptions attenuated the effectiveness of the policy.

Abstract
The different role of prenatal and postnatal exposure to tobacco smoke in respiratory outcomes in infants has not yet been clearly established. Our objective is to assess the effects of these exposures on the risk of respiratory outcomes during the first year of life of infants from a Spanish multicenter cohort study. A total of 2506 women were monitored until delivery. About 2039 infants made up the final population. The outcomes were caused by the occurrence of the following: otitis, cough persisting for more than 3 weeks, lower respiratory tract symptoms (wheezing or chestiness), and lower respiratory tract infections (bronchitis, bronchiolitis, or pneumonia). The relationship between prenatal and postnatal exposure and health outcomes was explored using logistic regression analysis. Maternal smoking during pregnancy increased the odds for wheezing (OR: 1.41, 95% CI: 0.99-2.01) and chestiness (OR: 1.46, 95% CI: 1.03-2.01). Postnatal exposure from fathers was associated with otitis (OR: 1.25, 95% CI: 1.01-1.54). Passive exposure at work of non-smoking mothers during pregnancy was related to cough (OR: 1.62, 95% CI: 1.05-2.51). Exposure to tobacco smoke was related to a higher risk of experiencing respiratory outcomes in young infants. Prenatal exposure was that most clearly associated with the respiratory outcomes analyzed.

Abstract
BACKGROUND: Arabs in Israel have high morbidity and mortality from diabetes and cardiovascular disease. Obesity is a risk factor for both conditions. OBJECTIVES: To investigate the prevalence of obesity (body mass index >30 kg/m2), subjects' knowledge and behaviors, and their reports on practices of health-care professionals regarding body weight among Arabs and Jews. METHODS: The study participants (n=880) were randomly sampled from the urban population of the Hadera district in Israel. Data on demographic, socioeconomic and lifestyle characteristics; reports on height, current body weight and body weight at the age of 18 years; knowledge and behavior; and health-care professionals' practices with regard to body weight were obtained by interview. Anthropometric measurements were performed subsequently. RESULTS: Information on BMI was available on 868 participants (49% Arabs, 49% women, median age 46 years). Although the median BMI did not differ significantly between Arabs and Jews at age 18, the prevalence of current obesity was 52% in Arab women compared to 31% in Jewish women (P < 0.001), and 25% in Arab men compared to 23% in Jewish men (P = 0.6). On multivariate analysis, obesity was significantly associated with age, BMI at the age of 18 years, leisure time physical activity and cigarette smoking, but not with ethnicity. Fewer Arabs reported measuring their body weight and Arab women were less frequently advised to maintain an active lifestyle. CONCLUSIONS: The high prevalence of obesity among Arab women may be explained by lifestyle characteristics. Prevention of obesity in Arabs should be directed at women and should start preferably before adulthood.
