Abstract
AIMS: To: (i) investigate the development of smoking and snus use among Norwegian adolescents, and (ii) describe the users in each group. DESIGN: Two population-based surveys with identical procedures in 2002 (response rate 91.0%) and 2010 (response rate 84.3%). SETTING: Norway. PARTICIPANTS: A total of 6217 respondents, aged 16-17 years. Abstract
OBJECTIVE: To examine social gradients in tobacco use in Argentina and Uruguay, using newly available directly comparable data sets. METHODS: Secondary analysis of Global Adult Tobacco Survey data from Argentina (N=6645) and Uruguay (N=5581). Social gradients in current tobacco use, exposure to secondhand smoke, and cessation attempt were examined with sex-stratified and age-adjusted logistic regression. RESULTS: Among men, there is evidence of higher odds of being a current smoker among respondents with lower levels of education, but the association is only statistically significant for respondents with less than primary education in Uruguay (OR=2.15, 95% CI 1.22 to 3.77). Similarly, women with lower levels of education have higher odds of being a current smoker in Uruguay. The association between education and exposure to secondhand smoke is broadly similar for both sexes in both countries, with generally higher odds among groups with low education, though the relationship is only statistically significant among men in Uruguay (OR=1.77, 95% CI 1.08 to 2.92). In both countries, respondents with lower levels of education in general have higher odds of having attempted to quit smoking in the past year, although these associations did not attain statistical significance. CONCLUSIONS: Social gradients in tobacco use, exposure to secondhand smoke and cessation attempts are broadly similar in both countries. Efforts to evaluate the long-term effects of tobacco control efforts in these countries should monitor how policies affect national averages, and the social gradients that are embedded in aggregate data.

Abstract
AIMS: To study the association between measured or perceived overweight in adolescence and subsequent uptake of cigarette smoking and of the Swedish smokeless tobacco 'snus' (oral moist snuff). DESIGN: Population-based prospective cohort study with 7 years' follow-up. SETTING: Self-administered questionnaires and school nurses' visits. PARTICIPANTS: A total of 2922 children of both sexes and mean age 11.6 years at recruitment, resident in the Stockholm region, Sweden. MEASUREMENTS: Tobacco use was self-reported at baseline and on six subsequent surveys. Subjects' height and weight were measured by school nurses during the first 4 years, self-reported thereafter. Overweight perception was self-reported at the age of 15 years. FINDINGS: Overweight and perception of overweight were not associated with subsequent uptake of either smoking or snus among males. Among females, overweight at baseline was associated with uptake of smoking [adjusted hazard ratio (HR) 1.34, 95% confidence interval (CI) 1.09-1.63], but not of snus. A similar pattern was found with overweight status during follow-up. Among girls with low-educated parents, overweight at baseline predicted the uptake of both snus and smoking. Among 15-year-old females who never used tobacco perceived overweight was associated with subsequent uptake of smoking (adjusted HR 1.71, 95% CI 1.20-2.46), but not of snus. CONCLUSIONS: In Sweden, adolescent girls with actual or perceived overweight are at increased risk to start smoking, while indications that this increased risk applies to smokeless tobacco (snus) are limited to girls of low socio-economic status.

Abstract
OBJECTIVE: To evaluate the shear bond strength of composite resin bonded to pretreated porcelain surfaces by use of a pulsed Nd:YAG laser irradiation and by use of 8% hydrofluoric etching. METHODS: Surfaces of porcelain were roughened with coarse diamonds until the opaque body porcelains were exposed. The samples were divided into four groups. Samples of the first group were etched with 8% hydrofluoric acid for 3 min by following the manufacter's instructions. Samples of the 2nd, 3rd and 4th groups were separately irradiated by a pulsed Nd:YAG laser of three energy parameters of 15 Hz, 40 mJ; 0.6 W; 15 Hz, 60 mJ, 0.9 W and 15 Hz, 80 mJ, 1.2 W different respectively, each specimen was irradiated for 1 min. Then, silane coupling agent was equally placed on the treated surfaces for 3 min, the activator was used on each surface. The adhesive was applied and light-cured. The composite resin material was finally bonded to the treated porcelain surface, forming a 3 mm x 3 mm x 2 mm resin mass. Samples were then tested for shear bond strength between the treated porcelain and composite resin by using a SWD-10 electric universal testing machine with a crosshead speed of 1 mm/min until fracture. At the end of testing, the data was evaluated by using a two-way analysis of variance (ANOVA) comparing the shear bond strength. On the other hand, characteristic morphology of laser irradiated surfaces of porcelain was prepared and observed by a scanning electron microscopy. RESULTS: The shear bond strength of laser groups of 0.6 W, 0.9 W, 1.2 W and etching group was respectively 8.61, 14.07, 11.22 and 13.47 MPa. There was no significantly difference among groups of laser 0.9 W, 1.2 W and the etching group. This suggested that irradiated on surfaces of porcelain by Nd:YAG laser at the above laser energy could attain the same effectiveness as hydrofluoric acid etching. But the shear bond strength of laser 0.6 W group was significantly lower than the etching group. It suggested that shear bond strength would obviously decrease when laser energy was 0.6 W or lower than this parameter, and had no use in treatment on porcelain surfaces comparing to acid etching. SEM showed that porcelain surfaces irradiated by Nd:YAG laser exhibited rough, umbilicate, lava crater-like structure. This structure helped to produce mechanical retention between resin composite and porcelain. CONCLUSION: The pulsed Nd:YAG laser could replace etching method to pretreat the fracture porcelain surfaces for bonding with composite resin at appropriate energy parameters.

Abstract
The objective of this study was to evaluate the effect of smoking on response to nonsurgical periodontal therapy using the primary outcome measure of bleeding on probing (BoP). An periodontist performed periodontal therapy on 11 smokers and 14 never smokers with periodontitis. Two examiners assessed visible plaque index, gingival bleeding index, probing pocket depth (PPD), BoP, suppuration on probing and clinical attachment level (CAL), at baseline and three months after therapy. BoP was categorized as 0 (absent), 1 (small bleeding point) and 2 (blood flow from the sulcus). Total BoP value was obtained by summing values of 1 and 2. All subjects had significant reductions in mean PPD and percentages of sites with BoP, with no difference between the groups. Only never smokers presented statistically significant CAL gain. BoP was significantly and consistently reduced at sites with initial PPDs of 1-3 mm and 4-6 mm in both groups. Abstract
In the title mol-ecular salt, C12H22N5(+).Cl(-), the cation is protonated at the dimethyl-substituted tertiary N atom. The piperidine ring adopts a chair conformation with the exocyclic N-C bond in an equatorial orientation. The dihedral angle between the piperidine ring (all atoms) and the pyrimidine ring is 14.00 (1) degrees . In the crystal, the ions are connected by N-Hcdots, three dots, centeredN hydrogen bonds, forming inversion dimers, which are further connected by N-Hcdots, three dots, centeredCl hydrogen bonds. Aromatic pi-pi stacking inter-actions [centroid-centroid separation = 3.4790 (9) A] are also observed in the structure.

Abstract
In the cyclo-hexene ring of the title compound, C23H20BrNO3S, the -(CH2)4- atoms are positionally disordered [occupancy ratio = 0.753 (6):0.247 (6)]. The ring has a half-chair conformation for both the major and minor components. The dihedral angles between the mean plane of the thio-phene ring and those of the benzene and phenyl rings are 35.2 (4) and 57.7 (3) degrees , respectively. The planes of the two aryl rings are twisted with respect to each other by 86.4 (6) degrees . In the mol-ecule, there is an O-Hcdots, three dots, centeredN hydrogen bond forming an S(6) ring motif. In the crystal, mol-ecules are linked via C-Hcdots, three dots, centeredO hydrogen bonds, forming chains parallel to [100].

Abstract
The title compound, C(22)H(23)BrN(4)O(3)S(2), crystallizes with two mol-ecules, A and B, in the asymmetric unit. In one of these, the meth-oxy group is disordered over two sets of sites in a 0.565 (9):0.435 (9) ratio. The benzene rings bridged by the sulfonamide group are tilted relative to each other by 37.4 (1) and 56.1 (1) degrees in mol-ecules A and B, respectively, while the dihedral angles between the sulfur-bridged pyrimidine and benzene rings are 72.4 (1) and 70.2 (1) degrees for A and B, respectively. The piperidine ring adopts a chair conformation in both mol-ecules. In the crystal, inversion dimers linked by pairs of N-Hcdots, three dots, centeredN hydrogen bonds occur for both A and B; the dimers are linked into [010] chains by C-Hcdots, three dots, centeredO hydrogen bonds. The crystal structure also features inversion-generated aromatic pi-pi stacking inter-actions between the pyrimidine rings for both mol-ecules [centroid-centroid distances = 3.412 (2) (mol-ecule A) and 3.396 (2) A (mol-ecule B)].

Abstract
INTRODUCTION: Gastroesophageal reflux disease (GERD) is probably one of the most prevalent diseases in the world that also compromises the quality of life of the affected significantly. Its incidence in Brazil is 12%, corresponding to 20 million individuals. OBJECTIVE: To update the GERD management and the new trends on diagnosis and treatment, reviewing the international and Brazilian experience on it. METHOD: The literature review was based on papers published on Medline/Pubmed, SciELO, Lilacs, Embase and Cochrane crossing the following headings: gastroesophageal reflux disease, diagnosis, clinical treatment, surgery, fundoplication. RESULTS: Various factors are involved on GERD physiopathology, the most important being the transient lower esophageal sphincter relaxation. Clinical manifestations are heartburn, regurgitation (typical symptoms), cough, chest pain, asthma, hoarseness and throat clearing (atypical symptoms), which may be followed or not by typical symptoms. GERD patients may present complications such as peptic stenosis, hemorrhage, and Barrett's esophagus, which is the most important predisposing factor to adenocarcinoma. The GERD diagnosis must be based on the anamnesis and the symptoms must be evaluated in terms of duration, intensity, frequency, triggering and relief factors, pattern of evolution and impact on the patient's quality of life. The diagnosis requires confirmation with different exams. The goal of the clinical treatment is to relieve the symptoms and surgical treatment is indicated for patients who require continued drug use, with intolerance to prolonged clinical treatment and with GERD complications. CONCLUSION: GERD is a major digestive health problem and affect 12% of Brazilian people. The anamnesis is fundamental for the diagnosis of GERD, with special analysis of the typical and atypical symptoms (duration, intensity, frequency, triggering and relief factors, evolution and impact on the life quality). High digestive endoscopy and esophageal pHmetry are the most sensitive diagnosctic methods. The clinical treatment is useful in controlling the symptoms; however, the great problem is keeping the patients asymptomatic over time. Surgical treatment is indicated for patients who required continued drug use, intolerant to the drugs and with complicated forms of GERD.

Abstract
An unusual case of penetrating trauma to the trunk by a water pipe during a car accident is presented. The only serious internal injury was an avulsion of the lower pole of the spleen, which was suture-repaired successfully after debridement. The line of management since admission is discussed. Follow-up after 1 year shows an asymptomatic and normally functioning young man.
