Abstract
Narghile smoking, a common habit among women in many non-Western societies, is assumed by the public to be minimally harmful. This study aims at identifying the effect of smoking narghiles during pregnancy on the weight of the newborn and other pregnancy outcomes. Three groups of pregnant women were interviewed in several hospitals in Lebanon between 1993 and 1995: 106 who smoked narghiles during their pregnancy, 277 who smoked cigarettes, and 512 who did not smoke. The adjusted mean birth weight of babies born to women who smoked one or more narghiles a day during pregnancy and to women who started smoking in the first trimester was more than 100 g less than that of babies born to nonsmokers (p < 0.1). The adjusted odds ratio of having babies with low birth weight (<2,500 g) among the narghile smokers was 1.89 (95% confidence interval (CI) 0.67-5.38). The risk increased to 2.62 (95% CI 0.90- 7.66) among those who started smoking narghiles in the first trimester. A stronger association and a dose-response relation were found among cigarette smokers. The association between narghile smoking and other pregnancy outcomes, especially Apgar score and respiratory distress, was also noticeable. Further research and a policy action to fight the misperception that narghile smoking is safe are both recommended.

Abstract
Narghile (water pipe, hookah, shisha, goza, hubble bubble, argeela) is a traditional method of tobacco use. In recent years, its use has increased worldwide, especially among young people. Narghile smoking, compared to cigarette smoking, can result in more smoke exposure and greater levels of carbon monoxide (CO). We present an acutely confused adolescent patient who had CO poisoning after narghile tobacco smoking. She presented with syncope and a carboxyhemoglobin level of 24% and was treated with hyperbaric oxygen. Five additional cases of CO poisoning after narghile smoking were identified during a literature search, with carboxyhemoglobin levels of 20 to 30%. Each patient was treated with oxygen supplementation and did well clinically. In light of the increasing popularity of narghile smoking, young patients presenting with unexplained confusion or nonspecific neurologic symptoms should be asked specifically about this exposure, followed by carboxyhemoglobin measurement.

Abstract
BACKGROUND: Waterpipe tobacco smoking has spread worldwide. However, the evaluation of scientific output in the field of waterpipe tobacco smoking has not been studied yet. The main objectives of this study were to analyze worldwide research output in the waterpipe tobacco smoking field, and to examine the authorship pattern and the citations retrieved from the Scopus database for over a decade. METHODS: Data from January 1, 2003 through December 31, 2012 were searched for documents with specific words regarding waterpipe tobacco smoking as "keywords" in the title. Scientific output was evaluated based on a methodology developed and used in other bibliometric studies: (a) total and trends of contributions in waterpipe tobacco smoking research between 2003 and 2012; (b) authorship patterns and research productivity; (c) collaboration patterns; (d) the citations received by the publications; and (e) areas of interest of the published papers. RESULTS: Worldwide there were 334 publications that met the criteria during the study period. The largest number of publications in waterpipe tobacco smoking were from the United States of America (USA) (33.5%), followed by Lebanon (15.3%), and France (10.5%). The total number of citations at the time of data analysis (October 18, 2013) was 4,352, with an average of 13 citations per document and a median (interquartile range) of 4.0 (1.0-16.0). The h-index of the retrieved documents was 34. The highest h-index by country was 27 for the USA, followed by 20 for Syrian Arab Republic and Lebanon. CONCLUSIONS: The present data reveal a promising rise and a good start for research activity in the field of waterpipe tobacco smoking. More effort is needed to bridge the gap in waterpipe smoking-based research and to promote better evaluation of waterpipe smoking, risks, health effects, or control services worldwide.

Abstract
BACKGROUND: Studies on the submaximal aerobic capacity of exclusive narghile smokers (ENS) seem necessary in view of effective prevention of cardiorespiratory diseases. The goal of the study was to assess, by 6-min walk test (6MWT) data, the submaximal aerobic capacity of ENS, to identify factors influencing their 6-min walk distance (6MWD), and to compare their data with those of a healthy non-smoker (HNS) group. METHODS: Seventy 20-60-y-old male ENS were included. Narghile use (narghile-years) and anthropometric, clinical, spirometric, and 6MWT data were collected. Univariate and multivariate analyses were used to identify factors influencing 6MWD. Data of a subgroup of 40-60-y-old ENS (n = 25) were compared with those of an age-matched HNS group (n = 53). RESULTS: The median (first to third quartile) for age and narghile use were 32 (26-43) and 17 (8-32) narghile-years, respectively. The profile of ENS performing the 6MWT was as follows: at the end of the 6MWT, 34% and 9% had a low heart rate ( 5/10, visual analog scale), respectively; 3% had an oxyhemoglobin saturation decrease of > 5 points during the test; and 20% had an abnormal 6MWD (less than the lower limit of the normal range). The factors that significantly influenced the 6MWD, explaining 38% of its variability, are included in the following equation: 6MWD (m) = 742.63 - 5.20 x body mass index (kg/m(2)) + 25.23 x FEV1 (L) - 0.44 x narghile use (narghile-years). Compared with HNS, the subgroup of ENS had a significantly lower 6MWD (98 +/- 7 vs 87 +/- 9% predicted, respectively). CONCLUSIONS: Narghile use may play a role in reducing submaximal aerobic capacity. The present study suggests that a program of pulmonary rehabilitation is an excellent axis to follow.

Abstract
A pilot study of narghile water-pipe smokers in a cafe in the Hamra neighborhood of Beirut, Lebanon, was conducted to develop a preliminary model of narghile water-pipe smoking behavior for use in laboratory smoking machine studies. The model is based on data gathered from smoking sessions of 30 min or longer duration from 52 smoker volunteers using a differential pressure puff topography instrument, as well as anonymous visual observations of 56 smokers in the same cafe. Results showed that the "average" water-pipe cafe smoking session consists of one hundred seventy-one 530-ml puffs of 2.6-s duration at a frequency of 2.8 puffs/min. The implications of this comparatively high-intensity puffing regimen on the production of toxic smoke constituents are discussed. © 2004 Elsevier Inc. All rights reserved.

Abstract
The biological effects of smoking water pipe on haemostasis and the eicosanoid system is unknown. Water pipe smoking is familiar to approximately 1 billion people around the world. Considering this quite impressive number, we investigated the potential effect of smoking the Narghile on oxidation injury by monitoring parameters of the (iso)eicosanoid system. Patients were allowed to smoke a water pipe once daily for 14 days. Blood was drawn from 7 healthy adult non-cigarette smoking male volunteers before and immediately after the first smoking of the water pipe and additionally after 6 hours. One and 2 weeks thereafter, blood was drawn again before and after smoking. A total of 7 blood samples was drawn during the study, and parameters of in vivo oxidation injury (8-epi-PGF2α, malondialdehyde [MDA]) and haemostasis (11-dehydro-thromboxane B2 [11-DH-TXB2]) were investigated. A single smoking session increased oxidation injury (8-epi-PGF2α: p = 0.03; MDA: p = 0.001) and 11-DH-TXB 2 (p = 0.00003) significantly, and repeated daily smoking induced a persistent long-lasting oxidation injury reflected by elevated prevalues but a smaller response to the actual water pipe smoke. These findings indicate a significant increase of in vivo oxidative stress by regular water pipe smoking. © 2003 Elsevier Inc. All rights reserved.

Abstract
BACKGROUND: Throughout the Eastern Mediterranean region (EMR), tobacco is used primarily in two forms: cigarette smoking and waterpipe (called narghile (nar-GIL-eh) in Syria) smoking. OBJECTIVE: To explore whether narghile smokers are different from cigarette smokers in how their smoking habits are embedded in their everyday lives. METHODS: One-to-one interviews with 16 adult narghile smokers and 16 adult cigarette smokers about their feelings, experiences and beliefs regarding their initiation, use patterns, and attempts to quit. FINDINGS: Narghile smokers found that narghile was a pleasurable social experience embedded in cultural rituals. By contrast, cigarette smokers saw their cigarette smoking as a mundane, oppressive, personal addiction. Narghile smokers generally started in their 20s and found that smoking narghile fostered a sense of togetherness and cultural identity, while cigarette smokers started in their early teens, males having started to becoming a "real man". Unlike cigarette smokers who felt stigmatised, narghile smokers generally felt that smoking narghile was socially accepted. Cigarette smokers believed that cigarettes were harmful to their health and harmful to those around them, but narghile smokers believed smoking narghile was relatively harmless to themselves or to others. Unlike cigarette smokers who used cigarettes to manage stress, narghile smokers used narghile for entertainment, leisure, and escape. However, frequent narghile smokers confessed that they felt addicted in much the same way as cigarette smokers. Cigarette smokers and narghile smokers viewed quitting as a matter of will and conviction. Most cigarette smokers had tried to quit. Very few narghile smokers had ever tried to quit, and most were not interested in quitting. Disturbingly, some cigarette smokers had tried to quit cigarettes by switching to smoking narghile, but later relapsed to smoking cigarettes. CONCLUSIONS: This exploratory study suggests that Syrian narghile smokers are different from Syrian cigarette smokers in their perceptions and beliefs about their smoking, and in their smoking patterns and lived experiences with tobacco. Our findings indicate that further in-depth research is need in the EMR to understand both modes of smoking to develop effective mode-specific prevention and cessation approaches. This study also raises concerns about a possible pattern where cigarette smokers are using narghile as a method for quitting cigarettes, and then relapsing.

Abstract
OBJECTIVES: Measuring the prevalence and determinants of narghile smoking among teenagers in schools in the southern suburbs (Dahia) of Beirut, Lebanon. METHODS: A simplified questionnaire was self-completed by intermediate and secondary students selected from private and public schools in Dahia the year 2001-2002. Variables obtained included details on narghile and cigarette smoking, sociodemographic characteristics of the student and his/her family, friendship networks, and beliefs regarding health. RESULTS : A total of 1461 students completed the questionnaire, of which 24% admitted regularly (> once/week) smoking the narghile and 14.4% occasionally, with a male predominance. The initiation was at about 13.6 yrs of age, but it varied from as low as 5 to 18 yrs. At least 28% of first narghile trial took place with a member of the immediate family, even though 58.2% initiated with a user friend. The monthly cost of using narghile did not exceed 7 USD for about half of narghile users in that area. CONCLUSIONS: Narghile use is a behavior gaining in importance in Lebanon in general, and in the Dahia area in particular. Its initiation among adolescents means that the probability of addiction is higher and the adverse health consequences are potentially more severe. It requires immediate interventional procedures including the launching of a campaign of national awareness for which hard data, such as those provided by this study, will be needed.
