The International League Against Epilepsy remains committed to collaborating with WHO to protect patient safety and treatment effectiveness. Substandard and falsified epilepsy medicines require a multi-layered approach as these products can cause inconsistent dosing, breakthrough seizures, status epilepticus, increased toxicity, and SUDEP risk, undermining IGAP goals. Three pillars are essential. Regulatory oversight: strengthen national authorities, harmonize GMP and bioequivalence standards, ensure batch testing, and expand systems like EMVS. Supply chain integrity: apply track-and-trace systems, follow Interpol’s six Ps, and create a global registry of suspect products. Education: inform clinicians, pharmacists, patients, and caregivers about risks. Efforts must balance safety with affordability to avoid limiting access
