This MMI statement is supported by PHM. The “World Together” decision called on MS to “prioritize equity” and “solidarity with all people and countries”. However, equity-promoting measures in the instrument are mostly voluntary. There are no binding obligations on technology transfer, support for geographically diversified manufacturing, debt relief during pandemics, and post-trial access and benefit sharing. The principle of common but differentiated responsibilities, and the right to enjoy the benefits of scientific progress are not explicitly recognised. The text includes no manufacturer commitments to WHO during outbreaks, thus risking escalation of outbreaks to PHEICs and pandemics. The highly qualified and voluntary language on manufacturer commitments is disappointing. As Covid-19 and mpox showed, voluntary language does not translate into early and equitable access to VTDs. We urge the IGWG to develop a PABS instrument that does not uncouple access to pathogens from benefit sharing measures.
