view Completed Health in Mountain Agenda (HMA) On 6-7 June 2024 the HMA policy dialogue was successfully organized: more than 90 participants from all stakeholders, President Administration, the regional and sub-regional authorities, PHC represenеatives , sectoral ministries and agencies, Village Health Committee, development partners. Dialogue three main blocks pivot the discussions: importance of health in the mountain agenda, health service delivery via strong PHC, the social, health determinants. The two panel discussions and an interaction session prompt particшpants engagement and brainstorming over inter and multi sectoral issues requiring the high level of collaboration. One of conclusions is to conduct the thorough assessment of situation with service delivery to generate evidence and support the Government reform actions. The comprehensive barrier assessment tool will be revised to enhance the scope and include at least 4 topics (PHC, HLMA, UHPR, Digitalization). glajman

WHO in coordination with World Bank planned the TA to MHIF to deliver DLIs 5,6 upon request from MoH and MHIF. The WHO part in the improvement of PHC financing and SGBP revision is supporting MHIF to conduct PHC service costing. MHIF created the PHC costing WG, the WHO international consultant visited the country in April 2023 and woks online with Costing WG. The 10 pilot FMCs representing different type of PHC organizations (urban/rural, rayon/oblast, population density, distance from centers) selected and approved by MHIF order, the costing methodology presented, adjusted and approved by the Costing WG. Support launch and implementation of the newly approved permanent regulation for price control for essential medicines  - an executive order of the MoH with responsibilities of involved stakeholders and timelines for implementation of the regulation issued by end of July 2023;
- a guideline for involved parties with detailed actions to implement the regulation is developed and circulated by end of July 2023;
- a series of informative meetings with applicants (business sector) to communicate the newly approved permanent regulation conducted by end of October 2023;

Support development of high quality standards for the health workforce Development of a sectoral qualifications framework for the health sector to ensure high-quality professional standards for the health workforce. At the moment, medical professional associations of the country are developing these professional standards in order to improve the accreditation and training system of medical and pharmaceutical staff and there is a need in providing them with developed health sectoral qualifications frameworks that meet all requirements of the National Qualifications System concept of the Kyrgyz Republic and Ministry of Health’s standards.

Improving financial protection through expanding access to medicines. This includes the development of Drug Regulatory Agency institutional strategy and capacity building, working on procurement and pricing to improve access to selected Essential Medicines, e.g. to address high burden from NCDs. Price regulation and practices are improved leading to decreased patient cost sharing to CVD related essential medicines in additional drug package. Additionally, the new regulation for the selection of medicines into the Additional Drug Package (ADP) is being implemented. The various forms required to launch this new process have been developed. The submission of applications in accordance with the new regulation is planned to start in January 2024.

Support to the scaling up and development of mechanisms of price regulation, as well as a public campaign on new prices for medicines and reimbursement model To date, prices for 355 medicine items have been registered. The next stage involves developing a monitoring plan to ensure market prices align with the registered prices. Additionally, it is planned to communicate the regulations to all involved stakeholders based on the communication plan. The automation of price extraction is also planned to be introduced with the support of the World Bank.

Strengthening of Single Payer Institutional Capacity Information Management system upgraded
, Upon request from MHIF to provide technical assistance and consultations of a tender winner IT company to develop a new Information Management System for MHIF (DB IMS), WHO supports the whole p[ack of supporting measures: facilitation of process of development of DB IMS, preparation of the section on IT development in MHIF, development of financing algorithm as part of DB IMS, effective communication between MHIF and an IT company and representing MHIF in technical discussions with an IT company. The development of financing algorithm as part of DB IMS, testing the software product at the stage of transferring the software package to the MHIF, integration with medical information systems in hospitals, integration of DB IMS and DRG grouper, development of the IS User’s Manual and training materials for MHIF and hospitals.

Providing tools for improving health financing towards strategic purchasing. TA provided to the multi phased introduction plan of new DRGs into the health financing system. , Full description of DRG development process during 5 years prepared and officially handed over to MHIF to be used as a Guidance for further work on DRG improvement. The new MHIF information management system developed based on new DRG system making the process of introduction of new DRG system in the case based payment irreversible. Within the finalization process the following aspects of comprehensive DRG system has been worked out:(1) the approaches to financing for short-term (one-day) stay of children, short-term (one-day) stay of pregnant women, emergency medical care (up to 3 hours), emergency medical care (from 3 to 24 hours); analysis of 2021-2022 data of TB hospitals and outpatient treatment; further development of new DRG model and improving the "Treated Case" information system. Series of training for MHIF territorial branches and hospitals on new DRG system conducted.

Health system strengthening MoH stewardship capacity to lead and implement state health reform program in a coordinated way is supported: Joint Annual Review of MoH and DPs conducted
, In 2023 MoH has requested support from WHO with development of the second phase action plan 2024-2030 (Action plan) of SHP based on the analysis of the action plan 2019-2023 implementation progress, lessons learnt during the pandemic. To support this process WHO worked with MoH to create the Inter-Sectoral Working Group (ISWG) and Expert Group (EG), prepare the outline for development of the SHP Action plan. On 30 March 2023, this Outline was presented by the MoH to development partners with a request for their support. SHP EG conducted analysis of implementation progress and the pandemic impact and lessons to take into account in the next phase action plan by the 9 SHP priority areas. The ISWG&EG retreat was organized on 12-15 October 2023 to put together the analysis conducted by each priority, discuss the cross-cutting enablers, prioritization of accelerators to prepare the first draft of SHP seven years action plan and amendments to SHP. The first ISWG meeting on 21 November 2023 approved eh SHP analysis for 2019-2023, the draft plans for priorities and the accelerators.

Strengthening financial protection (SDG 3.8.2) monitoring system Capacity building to produce data and monitor financial protection
, The WHO Barcelona Office for Health Systems Financing and WHO CO conducted the WHO Tracking health spending in Kyrgyzstan mission in March 2023. The mission focus area was tracking health spending and financial protection. Mission met the MoH, MHIF, MinFin, UN RC office and participated in the Health DPCC meeting. The mission findings were that the according to 2019 data, Kyrgyzstan has a smaller than average size of government allocation on health, the public spending on health in real per capita terms is roughly the same as a decade ago, health spending has been de-prioritized in government’s budget, and that low priority to health and limited fiscal space lead to low public spending on health as a share of GDP. WHO puts efforts into the institutionalization of SHA production for decision-making purposes. The WHO Barcelona office organized the 5th Sub-regional European meeting on tracking health spending using SHA 2011 methodology to improve health accounts production quality and discuss the ways to use the results to influence policy. Kyrgyzstan delegation included representatives of President Administration, MoH, MHIF, Ministry of Finance to engage all stakeholders important for the SHA 2011 calculation

The Health DPCC meeting held on 30 March 2023 was called upon the MoH request to inform DPs on big task in front of MoH to develop the new 7 years action plan for implementation of State Health Program "Healthy Person-Prosperous Country" 2019-2030. Strengthening of primary health care services - recommendations produced during the mission of the Kazak PHC Association are published and shared with national stakeholder and development partners

, Jointly with the WHO European PHC Center facilitated creation of the PHC Task Force as a driving force of reforms on wide spectrum of system components as well as a platform for open discussions of different topics and finding solutions for cross-cutting issues. PHC Task Force is assigned to deliver results against 9 ToRs namely (1) Revise the rules for enrollment of the population to PHC; (2) Revision of the legislative framework, regulations for PHC, including development of basic terms and definitions; (3) Revision of the SBP service package, PHC services package, assessment of compliance of providing SBP to the population at PHC level; (4) Regulations for PHC work organization (health profile, site passport, triage, mode and schedule of work, etc.); (5) Development and implementation of Referral system including standards, procedures and criteria for referring patients to other health care organizations for consultation of narrow specialists (outpatient type, specialized organizations), admission in secondary and tertiary levels hospitals, including the procedure for obtaining feedback from a family doctor, referral of women in labor and sick children, patients with acute CVD conditions; (6) Develop indicators for assessing activities, processes, outcomes at PHC level; (7) Implementation of e- medical patient record/card at PHC level, integrated into all levels of health care, for comprehensive patient management and delivery of integrated services; Implementation of modern information technologies (phone or online consultations, telemedicine, e- registration, recording, referral, referral of patients) in the activities of PHC HCOs to improve access and quality of services, as part of E-health development; (8) Costing of PHC services delivery, including SGBP; (9) Information and communication campaign to promote PHC. The final 2023 review of PHC Task Force accomplishment across the 9 ToRs conducted on 22 November 2023 identified significant progress and results in some areas as approval of quality measurement indictors for PHC, increase amount of nurses from 1 to 2 approval of Law “On the protection of health of citizens in the Kyrgyz Republic": This is fundamental law in healthcare, replacing the 10 sectoral laws eliminating duplications, fragmentations, contradictions and outdated norms; sets the objectives and principles, regulatory framework, legal relations, norms and mechanism for their implementation; introduced new modern concepts have been introduced: digital health, maternal and child health, infectious and non-communicable diseases; and stipulates rights and obligations of citizens in the field of health protection.

Inclusive, integrated, and multi-profile primary health care ready to spreadhead the health system response to the country's epidemiological burden MoH Kyrgyzstan was included in the 3nd course on ‘Actionable Primary Health Care Performance Measurement and Management’ organized jointly by the WHO Collaborating Centre Primary Healthcare Systems of the University of Amsterdam (the Netherlands) and WHO Regional Office for Europe. The MoH created the participants group of 19 specialist from MoH, MHIF, E-Health Center, Kyrgyz medical institute of retraining and continuous education, family doctors, DPs, participated in the first week of online training in March 2023, in the country -specific coaching sessions, including the one on the Diabetic Registry with the country best example from Croatia. The Kyrgyz team selected the diabetic indicators to develop eth country case. In November 2023 the second week of online training will take place.

Scaling up the Quality of care and patient safety in Kyrgyzstan recommended by the QOC assessment and further collaborative consultations QoC unit of MoH capacity strengthened
, WHO supported MoH with development of the Quality of Care and Patient Safety (QoC&PS)strategy and road map which has been approved on 4 May 2023. The Coordination Committee on QoC and PS with a function to lead implementation of QoC &PS strategy has been approved on 31st May 2023 # 657. WHO coordinated the QoC strategy development TA with World Bank and co-lead the Quality Strategy Consensus Building Workshop held on 11-13 January 2023 prior to approve of QoC&PS strategy. Kyrgyzstan was invited to become a member of WHO European Quality of Care & Patient Safety Focal Points Networks at its first organizational Joint Meeting organized by the WHO Athens Quality of Care and Patient Safety Office in May 2023, in Athens, Greece. The MoH representatives presented the QoC strategy among the 26 countries from the European region of WHO. the plans for next 6 months preliminary agreed to support MoH QoC unit capacity building.

Health in Mountain Agenda: Barrier Assessment to effective coverage with health services  The HMA policy dialogue supported the proposal of conducting assessment of the effective health services coverage in the mountains areas. The preparatory work started engaging 3 levels of organization to gain the synergy by combining resources and enhancing the barrier assessment handbook with the additional questionnaires on PHC, human resources, UHPR, digitalization. The assessment will generate evidence and identify supply- and demand-side barriers to health services, streamline the national discussion led by the Government toward the key barriers and issues to be addressed and foster the inter sectoral coordination and cooperation.

Promoting the PHC service costing function in MoH and MHIF    The result of PHC costing has been presented to MoH and MHIF, PHC costing working group in June 2024. Ongoing health financing reforms and changes in the management of MHIF affected the agreed plans to handover the PHC costing methodology to MHIF for the process institutionalization. The adjustment measures are being undertaken to work with MoH and MHIF new functions to facilitate understanding and acceptance the costing. Upon MoH/MHIF request the costing for narrow specialist services utilization at the PHC per speciality has been started to add to the PHC costing results. Coordinated with WB work on development of co-efficient system (gender/age, density, urban/rural) the costing results will be used to calculate the PHC per capita rate to ensure provision of services declared in the State Guaranteed Benefit Package (SGBP)

Support of the human resources for health information system’s education sector module. Comprehensive Health Labour Market Analysis (HLMA) was conducted in 2024 and is under finalization currently. This analysis provided a detailed mapping of data, stakeholders, and policies related to human resources for health (HRH) in Kyrgyzstan. The findings identified key shortcomings and outlined actionable steps to address them, focusing on rural family doctors and nurses who play a critical role in delivering PHC services. The analysis aims to inform the development of a strategic HRH document and corresponding policy dialogues in 2025, aligning with national health system objectives.

WHO leadership in coordination of development partners working in health for effective collaboration for UHC WHO strategic leadership in the health sector during 2024: Health DPCC meetings conducted followed by the minutes of meeting prepared and distributed.
