In response to the emerging needs to better manage Non-Communicable Diseases (NCD), the Palestinian Medical Relief Society (PMRS) designed a new phase for its Chronic Disease Program, which began in 2000 with the support of the Austrian  Development Cooperation. 
The new phase will be a continuation for the already achieved developments during the last four years, in which it will expand and strengthen the NCDs management services provided by the Chronic Disease Center in newly targeted primary health  care clinics.  According to the 2004  evaluation of the programme, PMRS was very successful in developing a health care model to prevent chronic diseases that could be easily implemented and integrated in all primary healthcare centers of  PMRS and other Palestinian health care providers. 
 
The proposed project for the years 2005-2007 builds on the progress achieved in the Chronic Disease Center in Ramallah, the Chronic Disease Mobile Clinics, and especially, the progress achieved in six pilot primary health clinics. The new phase  consists of four main components, which jointly contribute to building the capacity of Primary Healthcare Clinics to better manage NCDs. 
These components can be summarized as following:
- Capacity Building with regard to NCDs within the PMRS system;
- Developing a referral service system for NCD patients at the Chronic Disease Center (CDC) in Ramallah;
- Increasing community awareness for NCDs;
- Enhancing the policies and strategies of key-stakeholders to better manage NCDs, through the formulation of a National NCD Management Strategy;
