This is one of the anaemia prevention projects in Kharaz camp. The project was initiated after an MNP acceptability trial that lasted for two months. The project was designed based on UNHCR Operational Guidance on the Use of Special Nutritional Products to Reduce Micronutrient Deficiencies and Malnutrition in Refugee Population. The standard MNP formulation was slightly modified considering the local refugee context. The product name and the sachet design were selected in consultation with the refugee Somali community. The name 'DHEF' which means 'micronutrient' was selected . Promotional activities were done among the refugee community. Then, blanket distribution of MNP to refugee children was started in Kharaz camp and in the nine villages surrounding the camp. The project then expanded in order to include urban refugee children in urban areas in addition to children of the host community in areas were refugees gathered (namely Basateen and some areas in Sana'a).

The MNP currently distributed in the Bhutanese refugee camps is custom-formulated for the target population. Compared to the daily-recommended nutritional intake (RNI) for children less than five years of age, this formula has reduced vitamin A and iodine because the Bhutanese refugee children received fortified blended foods (WSB) fortified with these micronutrients in the general ration; reduced copper so not to exceed the 1 mg/day maximum recommended dosage for 1-3 year olds; extra vitamin K due to the inadequate consumption of vegetables among this population; doubled vitamin C content to enhance iron absorption. The MNP programme is following an alternate day schedule, where each child is to consume one full sachet mixed with solid or semi-solid food every other day. Currently, all eligible children aged 6-23 months receive 15 sachets of MNP on monthly basis, and all eligible children aged 24-59 months receive 30 sachets of MNP on bi-monthly basis. Mothers are advised to feed their children with regular family food and add the MNP directly on the child’s plate. Upon enrolment into the programme, AMDA nutrition workers provide instructions and educational sessions regarding the purpose, benefits, and proper use of MNP to the mothers and caretakers of all enrolled children. Regular monitoring and home visits are made by community health workers (CHWs), community nutrition workers (CNWs) and/or nutrition focal persons from AMDA/ UNHCR/WFP.

UNHCR’s response was to distribute a monthly ration of micro-nutrient powder, adapted to the refugee context, to approximately 4,000 children. An acceptability test was developed based on the “UNHCR Operational Guidance on the Use of Special Nutritional Products to Reduce Micronutrient Deficiencies and Malnutrition in Refugee Populations” and carried out in the two biggest camps in the Sahel Region of the country prior to scaling up the intervention to all camps and the urban centre of Bobo-Dioulasso. A communications strategy was developed focusing on the use of IEC materials adapted from UNHCR/WFP Nepal. Sensitisation included the distribution of leaflets and poster, sessions with women and care-givers during distributions and outreach work between distributions. Monthly post-distribution monitoring was conducted to assess adherence, utilisation, and knowledge.
Integrating certain products such as MNP into the diet of Malian refugee children requires longer term interventions on nutrition knowledge and behavioral change. Cultural beliefs and poor infant and young feeding practices are ingrained.

At the start, the MNP program was not well accepted by the camp population; before that, they were used to receiving milk (perceived like food) and fresh vegetables. The MNP program in pregnant and lacating women was stopped in 2012 due to very low acceptability. The acceptance of the MNPs in the other two groups increased over the years, however a large number of beneficiaries are still asking to change the MNP to something else.
