Sample survey data [ssd] self-administered All data processing (scanning, cleaning, editing, and weighting) was conducted at the US Centers for Disease Control. The overall response rate was 87.7%. The data is being distributed without warranty of any kind. The responsibility for the use of the data lies with the user.
